We have now removed the online form from our website as we are promoting patients signing up using the NHS App on their mobile phones or by visiting Log in - NHS App Online (service.nhs.uk) if you prefer to sign-up using a laptop or Desktop computer. Through the NHS App and website you will be able to verify yourself by uploading your own ID which will get approved and verified and then you will gain access to your online medical records and booking appointments/ordering repeat prescriptions. 

Patient Access is still a widely used app for accessing your records and if you prefer to sign up with this service we ask you to please come into the surgery with two forms of ID and complete one of our Online Access forms at the front desk reception at either Hook or Hartley Wintney.
