Social Prescribing

This is a way of linking patients in primary care with sources of support within the community. It provides GPs with a non-medical referral option that can operate alongside existing treatments to improve health and well-being.

Whitewater Garden for Health

Started in January 2017, this allotment in Hook gives patients the opportunity to help themselves improve their mental and physical health by being outdoors, being part of a team with the same goals and being active.

If you feel you would benefit from spending some time at our allotment in Hook, please speak to your GP.
