We are now offering travel vaccinations and consultations again with our Practice Nurse, Alex. 

Please complete the travel form below by either printing or completing in the surgery and return to reception. The travel nurse will then triage your form and be in touch to book your appointment. Once it is confirmed what vaccinations you need, we will send you a payment link to complete payment before your appointment if payment is required. If you do not pay before your appointment, your appointment may be cancelled.

Please do not give us the travel form any earlier than 6 months prior to your travel date.

Do you need vaccinations?

Our nurses can provide travel health advice and arrange for appropriate immunisations. Please arrange an appointment for a travel consultation at least 6-8 weeks before departure if possible. Vaccines need time to take effect and some may require a course over several weeks.

Travel Assessment

We offer a full travel advice and immunisation service. If you are travelling abroad please complete and submit the travel form in plenty of time (we recommend 6-8 weeks) before your journey.

Please complete our travel risk assessment form to the right and return to us at the surgery, where a nurse will assess what immunisations you may need. One of our team will then be in touch.

Travel Vaccination Prices

Travel advice is covered by the NHS; however some vaccinations and prescriptions are only available privately. Payment for non-NHS services must be made prior to your first vaccination.
